Changes to your CPP:

Employees over the age of 65 who decide that they no longer wish to contribute to the Canada Pension Plan or if you chose not to contribute in the past but wish to start again you must complete the Canada Pension Plan Contribution Exemption Request Form.  Please download the "CPP Exemption" form below, complete it and bring it into the office with proof that you are in fact collecting Canada Pension.  As a courtesy, we will mail the original copy to the Winnipeg Tax Centre for you.

Changes to your Personal / Federal Tax Deductions:

If you wish to increase your income tax exemption, or pay more income tax per pay through payroll deductions, please download and complete the "TD1 Tax Form" and the "TD1ON Tax Form" and bring them into the office.

Vacation Pay Requests:

If you wish to request your vacation pay, please complete the attached vacation pay request form and submit it to the office no later than 7 days prior to the requested deposit date.  If you are taking time off, please be sure to have your site supervisor sign off on the time requested. Barber-Collins Security tracks vacation time taken, so if you are taking time off, please be sure to have your site supervisor complete this section.

How to download the forms:

Please note that the forms that end in .pdf and have no "submit" button require a few extra steps:

1. Fill out the form right on your computer screen

2. Save it to your computer.  When you save, put your last name in front of the existing filename (example "SmithVacationPay.pdf")

3. Email the saved document as an attachement to:  payroll@barbercollins.ca

You are also welcome to print the form, complete it manually and deliver it to the office directly or send it in by fax to 519-745-3554
